What’s Needed

Patient’s medical records are a key component of our proprietary medical review. The evidence of medical violations used to successfully dispute & dismiss hospital bills are captured & preserved in these legal records.
Medical Record
  1. Google the hospital and call the phone number listed online (Do NOT call the number on the bill)
  2. Ask the operator to connect you to the “medical records department”.
  3. Request a complete legal copy of the medical record (NOT just a summary). The medical record includes your actual medical data from the hospital visit such as Physical exam reports, surgery reports, radiology & laboratory test results, etc.
Itemized Bill
  1. Call the phone number on your billing statement and request an itemized bill. It is a more in depth bill which includes details for each item of service. Hospitals are required to provide an itemized bill if you request it.
