- Google the hospital and call the phone number listed online (Do NOT call the number on the bill)
- Ask the operator to connect you to the “medical records department”.
- Request a complete legal copy of the medical record (NOT just a summary). The medical record includes your actual medical data from the hospital visit such as Physical exam reports, surgery reports, radiology & laboratory test results, etc.
- Call the phone number on your billing statement and request an itemized bill. It is a more in depth bill which includes details for each item of service. Hospitals are required to provide an itemized bill if you request it.
